I have been fully vaccinated but will continue to wear a mask when I arrive at your home. I ask that all who wish to be present during the process wear masks as well.
I will go over the entire euthanasia process via phone conversation or email correspondence in order to increase efficiency during the visit and minimize exposure time.
A consent form can be printed from my website to fill out in advance of my visit. This can be signed and returned via email or presented to me in person. If you are not able to access a consent form, I will present one at the beginning of my visit for you to fill out.
If you are paying with a check or cash, please have the total amount prepared in advance.
